Overview: The Psychiatrist on the ACT/FACT Team is the lead clinician on an interdisciplinary team serving adults with serious mental health diagnoses, substance use disorders, and co-morbid medical conditions. The Psychiatrist works collaboratively with the Program Director and staff to support the care and treatment of the program participants. Our clients have histories of trauma, homelessness, incarceration, long term hospitalizations and are primarily people of color who are victims of systemic racism. 80% of the work is conducted in the field, where our client live and spend their time.
Responsibilities:
Conduct Psychiatric Assessments and Evaluations
Prescribe, Monitor and Administer Psychotropic Medications
Provide crisis intervention services to clients during regular business hours and consultation during off-hours.
Lead/Participate in clinical case conferences, team meetings, High Risk conference, quality improvement, evidence-based practice implementation and policy/procedure development.
Oversee the work Psychiatric Nurse Practitioner, Registered and Licensed Practical Nurse(s), including prescribing practices, clinical assessment and diagnosis, documentation of services.
Communicate with Department of Health and Mental Hygiene Assisted Outpatient Treatment (AOT) responding to requests for information about the status of consumers subject to AOT orders
Assess and monitor the physical health of clients
Complete all clinical documentation as required by payers, funders, and regulatory authorities via an Electronic Health Record.
Coordinate care with family and other community treatment providers or collaterals including, but not limited to: hospitals, housing providers, shelter staff, criminal justice personnel, AOT, and primary health care providers.
Complete all administrative paperwork required
