The Licensed Practical Nurse will work on NY State Office of Mental Health funded care team of mental health professionals assisting clients in a permanent housing transitioning from homelessness to housing in the community

Responsibilities:
– Provide direct patient care, including administering medications, monitoring vital signs, and assisting with activities of daily living for our men shelter
– Collaborate with the healthcare team to develop and implement individualized care plans.
– Monitor and document patient progress, symptoms, and changes in condition.
– Assist with medical procedures and treatments, such as diabetes.
– Educate clients on healthcare procedures, medications, and self-care techniques.
-Have flexibility to work a schedule that includes evenings and weekends and ensure compliance with all regulatory standards and protocols.
