SCOPE OF ROLE: Provides support, education and guidance in order to assure healthy nutritional meals in the Community Transitional Programs.
RESPONSIBILITIES:
- Organize and plan menus for the person served in our care, in compliance with the New
- York City Food Standards.
- Support and ensure compliance with food and nutrition requirements within Behavioral
- Health Division
- Assist in reviewing and updating menus as needed for corrective remedies to ensure strict adherence to established standards.
- Review menus to assure they are in alignment with nutritional guideline and standard.
- Manages ongoing nutrition services that meet the needs of the person served by evaluating their nutritional needs.
- Engage, assess and refer high-risk person served to other health-related and social
- services, as necessary.
- Tracks high-risk participants’ progress in improving their health and document for
- Prepare and recommend meals for person served with special dietary needs.
- Actively participates in the interdisciplinary team providing recommendations and
- education based on nutrition expertise.
- Develop and implement high risk care plans and oversee high risk care to all participants.
- Develop educational and training program and materials for the person served and staff.

REQUIREMENTS:
REQUIRED EDUCATION AND EXPERIENCE
- Master Degree in Food & Nutrition from an accredited institution
- Certification with New York State as a Certified Dietitian/Nutritionist (CDN)
- 4 Years of experience
PREFERRED QUALIFICATIONS:
- Able to lead and effectively facilitate groups
- Strong interpersonal, organizational, verbal and written communication skills.
- Able to lead and effectively facilitate groups
- Ability to develop a collaborative, therapeutic alliance with individuals
- Basic computer skills (e.g. MS Office suite and Windows)
Job Type: Full-time
Pay: $95,000.00 – $100,000.00 per year
